Neighborhood care

Caring, nursing and guiding
Home care / community care is care, nursing or support that is delivered to your home. Guardian Zorg strives for flexibility in order to respond quickly and professionally to the changing support needs of its clients, with the aim of allowing you to retain your own control in your home environment for as long as possible.

Personal care
Guardian Care can help you get out of bed, washing, showering, dressing or using compression stockings. We support you with the meals and also help you to bed. In short, we offer help with all activities of daily life (ADL) and we fully adapt to your needs and wishes.

Nursing
We nurse in your own environment in case of illness or disability, regardless of the duration or intensity of the assistance. Competent employees perform nursing operations, such as wound care, injecting and dispensing medication. Our nurses can advise you on, for example, medication use, medical aids and facilities.
Our nursing at home can also be used in the final phase of life.

Accompaniment
We offer support and guidance in case of illness, loneliness, light memory problems or confusion. You can think of activities such as shopping, walking, visiting museums or an event visit. Guidance is offered to promote or preserve independence, so that you can stay in your own environment for as long as possible.

Bridging care

Customized bridging solution!
Sometimes there is no room available in the care or nursing home of your choice. Many nursing homes use a waiting list where it can take months before a room is available for you. In this case bridging care is a good alternative for you.

Guardian care provides you with temporary bridging care, until the moment you can transfer to the place of your choice. In the meantime you will receive care in your own familiar environment.

In an intake interview with you and one of our healthcare professionals, we will look for a suitable bridging solution.

Palliative and terminal care

The final stage of life in a familiar environment
Palliative care is focused on the quality of the final stage of life. Guardian care offers palliative and terminal care at home. Our goal is to let you spend your last phase in your own familiar environment and strive to provide support and care for you and your loved ones with the highest possible quality of life.

For care in the final phase of life, Guardian Zorg distinguishes between terminal and palliative care. In both cases, care is provided by a small and trusted team to ensure that the final phase of life runs as smoothly as possible. The care provision goes according to the patient's own wishes and choices. Our goal is to give the highest possible quality of life, as long as this is possible.

In cooperation with you, a select team of care providers is put together that can provide tailor-made palliative and terminal care.

Night care
Guardian care also offers night care as a supplement to day and evening care. There is then a care provider present to you that you can call on at night. Night care can be used flexibly 7 days a week.

Private care

Take care in your own hands
When regular home care is not sufficient or not at all possible, private care can offer a solution for you. Guardian care also offers private home care or a combination of these.

Are you afraid to stand alone in the night? can you, as a partner or informal carer, no longer only take care of your partner because of overload or lack of expertise? Night care can offer a solution for this.

In order to ensure that the private contribution does not become unbearable or does not fall into your budget at all, you can appeal to a Personal Bound Budget (PGB) from the Long-Term Care Act (WZZ).

24-hour care

24 hours a day, 7 days a week
In some cases there is a need for care or nursing at home day and night. For example, by a physical limitation, dementia, to relieve you, your partner or informal caregiver. In that case, there is the possibility of having a carer or nurse 24 hours a day and even 7 days a week for home support.

When you opt for 24-hour care, Guardian Zorg, in consultation with you, will put together a select team of professional caregivers who will provide you with attention, love and expertise.
